A legal description of the property for the establishment of the bank

A description of geographic boundaries of the proposed banking development district which specifi cally identifi es the location of the proposed banking branch within the proposed BDD.

Documentation of all current zoning classifications within the proposed BDD and the boundaries of each identified zoning area. Attachment files must not exceed 250 MB.

A narrative description of natural or man-made geographic barriers that could impede physical access to existing bank services.

A description of public transportation and major roadways: maps or diagrams may be included.

Socioeconomic Information

Please indicate the number of residents of the proposed BDD.

Provide detailed demographic and income information describing the population of the proposed BDD, using (where applicable) either the most recent United States Census Bureau data or the most recent Department of Housing and Urban Development (HUD) annual income estimates:

Provide a description of all banking and financial education programs that will be provided by the branch directly or in partnership with a CBO.

Identify specific banking services that are not being provided in the proposed development district at the time of the application.

Identify the number and location of existing sites of banking services within the proposed development district and within

a reasonable distance (i.e., the travel distance to available full-service banking facilities to the unbanked or underbanked

community that may be conveniently accessed by means of private transportation or safe, affordable and reliable public

transportation).

Identification and location of all full‑service branches of all depository institutions.

Identification and location of all ATMs that offer deposit services.

Identification and location of existing alternatives to providers of banking services including, but not limited to, currency exchanges, pawnbrokers, money transmitters, and payday lenders within the area.

Location of deposit-taking remote service facilities (not including those located at full-service branches).

Location of non-deposit-taking remote service facilities (not including those located at full-service branches).

Provide a narrative description of the distances and travel times from the proposed site to:

Nearby banking institutions or alternate bank services providers within the proposed BDD.

Nearby banking institutions or alternate bank services providers within a reasonable distance (see definition in V.2., above).

Additional Information Requests

Provide a narrative description of why this branch would be economically viable if a deposit of public funds was made: Include statement of limitations on the types of public funds that the bank would be willing to accept.

Specify if the proposed BDD is located in an area that is designated as an enterprise, empowerment zone or received any similar designation from any federal, state or local government entity.

The applicant depository institution must affirm that it will not establish the proposed BDD with the intent to close or relocate any existing nearby branch(es).

Provide documentation of the most recent rating given to the depository institution under the Illinois Community Reinvestment Act (ILCRA), if a rating is available. Attachment files must not exceed 250 MB.

Provide a description of the criteria generally used to determine whether to continue maintaining a branch in operation, and for the branch proposed in the application: Would the branch be evaluated under any different criteria?
