Requirements for Reactivating Your License

This webpage is only available to the following professions: Architect, Barber, Clinical or Prescribing Psychologist, Clinical or Licensed Professional Counselor, Clinical or Licensed Social Worker, Cosmetologist, Marriage & Family Therapist, Nail Technician, Salon Shop, Pharmacist, Pharmacy Technician, Physician, Professional Engineer, Registered Nurse, or Licensed Practical Nurse looking to reactivate their licenses.

If your license needs to be reinstated or restored, please use the chart below to identify what fee amount is owed and which application to use.

The first step is to identify the date your license expired, which will inform you of the fee that is due to reinstate or restore the license. For individuals who need to check their license expiration date, please use our license look up tool here.

As you follow the chart across, you will notice the fee amount is based on when you submit the payment (before or after the renewal period).

The last column with links will inform you what application you need to submit.

The last step will be to print the application and instructions, fill them out, and mail them and the fee to the Department.
