Name/Address Change Request
IDFPR requires each licensee to keep the agency notified of his or her current address, email address, and phone number.
Name Change
Currently, name changes CANNOT be completed via the online form or through your online portal. If your name has changed, you must submit this form to the Department and include COPIES of documentation of the name change (marriage license, court order, or divorce decree).
Address Change

NOTE: This address change form is NOT for international residents. International residents must submit the paper form, which can be found here.
Address changes made via this online form are ONLY available to licensees in an “Active” status. You may verify your license status by using IDFPR’s License Look-Up tool. Again, once you have verified your status as “active”, please use this online form.
If you do not have an online account, are an international resident, or a licensee with an “inactive” or “non-renewed” status, please use this form here to change your address.
