Personal Information Change Request
All licensees are required to keep the Illinois Department of Financial and Professional Regulation (IDFPR) informed of their current name, mailing address, email address, and telephone number.
To request an update to your personal information, please complete the Personal Information Change Request Form and upload the required supporting documentation as outlined on the form. Acceptable documentation may include a marriage license, court order, divorce decree, or driver’s license, depending on the type of change requested.
